Patient Forms
CWH employs a secure, encrypted electronic medical record system to manage all of our patient information. All new patients will be asked to complete our electronic registration process in the office via an easy-to-use electronic tablet. Our staff is available to help anyone requiring special assistance. There is no need to complete any paperwork prior to your visit; however a copy of the forms requiring signature is available here for anyone who wants to review the forms prior to their visit.
Patient Centered Medical Home (PCMH)
CWH is pleased to announce that we were among a select group of practices chosen to participate in the State of Maryland’s Patient Centered Medical Home Project, a program designed to improve the quality of health care delivery in Maryland. As a participant of the program, CWH will undergo a rigorous practice improvement process to deliver higher quality, more efficient care to our patients, and will apply for recognition through the National Committee for Quality Assurance. We are very pleased that the State of Maryland selected CWH for this great honor.
